Mindspan GUIDE is a free Medicare program built specifically to support families and caregivers navigating dementia - at no cost to you or your loved one.
• No out-of-pocket costs 80 hours of paid in-home respite care for Caregivers
• Monthly check-ins and care plan updates
• 1:1 access to care navigators trained in dementia
• Support for caregivers, behavioral issues, and clinical follow-up
• Integrated access to advanced diagnostics, therapies, and referrals
Your loved one may be eligible if they:
• Are enrolled in traditional Medicare (Parts A & B)
• Have a diagnosis of Alzheimer's disease or a related dementia
• Are not currently enrolled in a Medicare Advantage or hospice plan
•Have a primary caregiver who can participate in their care.
