Mindspan operates the CMS GUIDE Program, a new model of dementia care designed to provide members with comprehensive support.
This program is for individuals with a confirmed diagnosis of dementia who wish to continue living at home or in a community-based setting, like assisted living. To qualify, a person must be enrolled in traditional Medicare Part A and Part B. Please note that individuals enrolled in Medicare Advantage plans, PACE programs, or those residing in a long-term nursing home or receiving hospice care are not eligible. For those who qualify, membership includes a dedicated care team for seamless coordination of services, 24/7 support for any questions or urgent needs, and specialized training and education to equip their unpaid caregivers.
